ENABLING JOAN TO REHABILITATE AFTER A FALL

Joan suffers with hypertension which makes her at risk of falling given her poor mobility and age-related frailty.

Sadly, Joan suffered a severe fall which resulted in her breaking her arm and being admitted into hospital.  Joan’s family did not live nearby and were concerned about her recovery.  They really did not want her to go into a care home.

Prestige Nursing & Care were asked by Joan’s local authority social services team to put a care and support package in place.  They wanted Joan to get the support she needed with her mobility, personal care and help around the house, so she could focus on her rehabilitation.  A team of carers was identified to care for Joan who had completed  moving and handling and medications training, so they were equipped with the skills to support her to live well at home.

The team worked closely with other healthcare professionals involved in Joan’s care, to ensure a holistic plan of care was created.  This included a comprehensive physiotherapy programme and recovery exercises that the care team could support Joan with.

The team gently encouraged Joan to complete her physiotherapy programme to achieve the outcomes required by her healthcare team.  Progress was effectively monitored by the care team and communicated to healthcare and social care professionals involved in her care.  Joan was able to rehabilitate speedily with our care, resulting in a reduction of visits needed to support her each day.  She can now live independently in her own home, with minimal visits.  Her family are thankful that she did not need to move into a care home to get the care she needed.
