What is home from hospital care?
Home from hospital care provides temporary or ongoing support for people recovering at home after a stay in hospital. The level of support required will vary depending on a person’s condition, mobility and overall health.
Support may include:
- Assistance with personal care and hygiene
- Medication reminders and support
- Meal preparation and hydration support
- Mobility assistance and moving safely around the home
- Companionship and emotional reassurance
- Support following surgery or illness
- Help attending follow up appointments
- Assistance with household tasks and daily routines
- Specialist care for complex or long term conditions
Our care services are designed to help reduce stress during recovery while promoting comfort, dignity and independence at home.
Arranging care after a hospital stay
Before you or your loved one leaves hospital, we can work closely with hospital discharge teams, GPs and community healthcare professionals to understand the level of care and support required. Together, we will create a personalised care plan built around individual needs, routines and recovery goals. We can also help families understand what funding or financial support may be available.
Our responsive local teams can often arrange care quickly, helping provide peace of mind during what can be a challenging time.

Supporting recovery and independence
Recovering at home can often provide greater comfort and reassurance than remaining in hospital for longer than necessary. With the right support in place, many people feel more relaxed and confident in familiar surroundings.
Our carers provide compassionate support that encourages independence wherever possible, while ensuring safety and wellbeing remain a priority.
As recovery progresses, care can be adjusted to suit changing needs, whether that means increasing support temporarily or gradually reducing care as confidence returns.
Collaborating and partnering to care for you
We work collaboratively with other healthcare professionals, multidisciplinary teams, commissioning groups and other therapy providers involved in our client’s care to ensure an integrated and holistic approach to nursing care we provide. Family involvement is central to our approach to providing nurse-led care at home and our care teams will ensure they receive high levels of support, as and when they need it. The level of care provided is planned in partnership with everyone involved in a client’s care. We know through our experience this ensures goals are reached and outcomes are improved while allowing our clients to receive the very best care.

What does our home from hospital service include?

Quality assurance through regulated service provision
Prestige Nursing & Care is not an agency. Unlike a homecare agency, we provide a fully managed and regulated nurse-led care at home service, which means our clients, their families and our professional partners can be reassured that the care and support we provide is of the very highest quality.
Our service is inspected by the Care Quality Commission (CQC) in England and the Scottish Care Inspectorate (SCI) in Scotland. We are proud that 96% of our inspected services have been rated as providing a ‘Good’ service, with 4% rated ‘Outstanding’ and all our services in Scotland have been awarded a five-star rating.
Our accredited service for complex care, which meets local and national quality standards and ensures a consistent approach to the care we provide, provides peace of mind and reassurance that our service is of the highest quality.
Flexible care tailored to your needs
Our trusted and established home from hospital care service can be provided in the following ways for someone needing nurse-led care:

Hourly care
is for those who need care and support with daily living or clinical care at certain times during the day. Nurse-led care at home can be provided from a 30 minute visit, to as many hours/visits throughout the day you require, one day or several days of the week depending on your care and support needs. For those who have complex needs, hourly care can be provided over a 24-hour period.

Live-in care
for when a permanent care arrangement is needed with greater levels of support around the clock to ensure a person can live well at home. Through our family of leading homecare brands in the UK, we are able to offer award-winning live-in care rated ‘Outstanding’ by The Care Quality Commission (CQC).

Respite care
for those needing a short-term arrangement after hospital discharge to rehabilitate or when a family carer needs a break from their caring role. It is also an ideal service for families who wish to assess whether care at home is the right care choice for them.
Responsive local teams with nationwide coverage
We provide high quality homecare services throughout England and Scotland, supporting individuals and families in communities nationwide.
Our local teams work closely with healthcare professionals, multidisciplinary teams and community services to ensure a joined up approach to care and recovery.
With over 80 years of experience, we understand the importance of delivering care that helps people feel safe, supported and comfortable in their own home.

Cost of home from hospital care
The cost of home from hospital care will depend on the level of support required and how often care is needed. Every care package is tailored to individual recovery needs, whether that involves short daily visits, overnight support or more comprehensive care.
Before care begins, our experienced team will carry out an assessment at home or in hospital to understand the support required and recommend the most suitable care plan.
Depending on your circumstances, you may be eligible for healthcare funding or social care support to help contribute towards the cost of care.
Our team can provide clear, impartial advice to help you understand the options available.
A trusted hand when you’re coming home
The fully managed service we provide is highly responsive and our experts local to you can put care packages in place quickly, working in collaboration with local healthcare professionals, multidisciplinary teams, therapeutic care providers and other community support services. We know this holistic approach ensures our clients get the best out of life, for all of life.
