WHAT IS 24-HOUR HOME CARE?

24-hour care at home is provided to people who are either living with complex needs who need nurse-led care, or who are living with a condition that impacts their ability to live well and need condition-led care.  For generations of people our 24-hour home care offers continuous support provided by a team of carers who get to really understand your needs and are expertly trained to provide high-quality complex care or paediatric care in the comfort of your own home.

A small team of reliable carers will care for you on a rotation to ensure you get the right care, at the right time.  Whilst different shift patterns are offered, most clients benefit from carers working eight or 12 hours shifts over a 24-hour period.

A range of options is offered if you need care throughout the night, including our sleep on-call service, which means a carer will sleep during the night, but is there to assist should you need them.  Waking nights is offered to those who need night-time assistance, such as repositioning in bed, managing a ventilation machine or administering medications.

WHEN IS 24-HOUR CARE NEEDED?

If you are an older person faced with increasing care needs, or you are living with a progressive condition, or disability or you have sadly experienced an accident, you may wish to consider 24-hour care.  For those with complex needs and require nurse-led care at home for interventions like airway management, our 24-hour care ensures clinical needs are met around the clock.

If you are receiving complex care at home, the carers we choose to support your needs will have at least six months care or health sector experience before they come to care for you. In addition to completing our induction training programme, which goes above mandatory requirements they are trained by our registered nurses, clinical experts and other healthcare professionals involved in providing care to a specific client.  This hands-on nurse training means our carers can competently undertake a range of clinical interventions. CLINICAL EXCELLENCE AT PRESTIGE NURSING & CARE

Clinical excellence is at the heart of our culture and approach to service delivery at Prestige Nursing & Care. Our nurse-led home care service is monitored and supported by our experienced regional nursing teams.  Our team of qualified regional nurses are registered with the Nursing and Midwifery Council (NMC). Each holds a registered nursing qualification in at least one nursing specialism, including general adult nursing, learning and disability nursing, mental health nursing and children’s nursing. They enable the planning and delivery of safe and effective complex care to our clients, that is focused on ensuring the best possible care experience for our clients and their families.

Our nurses work in partnership with our care co-ordinators, carers, clients and families to identify specific care needs and plan the most appropriate care provision, whilst ensuring all our carers have the skills they need to deliver high standards of nurse-led care. John is a 58-year-old who has difficulties in maintaining electrolyte balance orally due to fluid loss.

He requires 24/7 RGN care to administer sodium chloride distal flushes every two hours over a 24-hour period, as well as managing a very complicated double stoma. John had been in hospital for just over a year prior to Prestige Nursing & Care being asked to care for him and securing a successful discharge back home.

The team is led by a lead nurse who provides clinical oversight as well as bespoke, tailored training to the nurse team that provides his care. In addition to support from a registered nurse, we also introduced a team of carers to support his domestic duties which significantly eased John's assimilation back into family life.  Not only have our team of nurses played a pivotal role in meeting John’s complex needs, but they have also been instrumental in his reablement at home.  Our lead nurse has worked closely with various multi-disciplinary teams involved in John's care, particularly with his physiotherapist which has enabled John to become more independent.  He has been able to continue with his hobbies and interests, which he has been unable to do prior to going into hospital.

The CCG Case Manager responsible for John’s care provision commented: “I visited John for the first time to complete his three-month review.  I was asked by the family to pass on their thanks and positive feedback about the nurses and carers looking after John.  They have total reassurance that the care is organised efficiently, is delivered by the same team offering them continuity and are pleased to know a registered nurse is always on shift to guide clinical interventions.”

COSTS OF 24-HOUR CARE AT HOME

The cost of 24-hour home care is dependent on the level of care and support you require, with the overall cost determined by the number of visits and frequency of visits to you at home. Our experts will conduct a comprehensive assessment of your needs at your home, or in hospital if you are awaiting discharge to truly understand what you need and therefore how much 24-hour home care will cost.

We know that our 24-hour care costs are very competitive when compared with other 24-hour home care providers.  It is worth noting that when comparing the cost of 24-hour care with a care package in a care home, that the fee you pay for 24-hour care at home is actually for the care and support you receive.  In a care home your weekly fee mainly covers your accommodation, food and social activities with the care package being only a small part of what you pay.You may be entitled to either healthcare funding, social care funding or Government funding to contribute to the cost of 24-hour home care.  If you are faced with having to finance your own home care (known as self-funding care), there are initiatives available to you, including care fees annuities or equity release schemes that could provide the funds you need for your care and support.

FINDING THE RIGHT 24-HOUR CARE PROVIDER
