COMPLEX NURSE-LED CARE AT HOME IN DERBY

Our established nurse-led service cares for generations of people – from babies who require paediatric care at home, to children with learning disabilities through to adolescents who have had a life-changing accident to adults and older people living with neurological conditions or other physical disabilities.  Our care is aimed to facilitate independence, social interaction and emotional well-being whilst supporting people to fulfil their potential and live a purposeful and meaningful life.

We are experts in the specialist care of conditions that impact a person’s ability to live well, including neurological conditions like dementia, Parkinson’s or motor neurone disease, physical disabilities such as muscular dystrophy and cerebral palsy, and palliative care for those who are nearing the end of their life.  We provide nurse-led clinical interventions including respiratory and airway support, like tracheostomy management, feeding support including PEG feeding and stoma and catheter care.

CLINICAL EXCELLENCE AT PRESTIGE NURSING & CARE

Clinical excellence is at the heart of our culture and approach to service delivery at Prestige Nursing & Care. Our nurse-led home care service is monitored and supported by our experienced regional nursing teams.  Our team of qualified regional nurses are registered with the Nursing and Midwifery Council (NMC). Each holds a registered nursing qualification in at least one nursing specialism, including general adult nursing, learning and disability nursing, mental health nursing and children’s nursing. They enable the planning and delivery of safe and effective complex care to our clients, that is focused on ensuring the best possible care experience for our clients and their families.

Our nurses work in partnership with our care co-ordinators, carers, clients and families to identify specific care needs and plan the most appropriate care provision, whilst ensuring all our carers have the skills they need to deliver high standards of nurse-led care. They also provide families with on-going advice and continual support to ensure they feel reassured about the service we provide.

Our trusted and established care at home service in Derby and across the East Midlands can be provided in the following ways:

Hourly care / visiting care

As it is sometimes referred is for those who need care and support with tasks of daily living or clinical care at certain times during the day. Care at home can be provided from a 30 minute visit, to as many hours/visits throughout the day you require, one day or several days of the week depending on your care and support needs. For those who have complex needs, hourly care can be provided over a 24-hour period.

We work collaboratively with our clients and their families to develop a thorough care and support plan that accurately reflects a person’s holistic needs.  We also adopt a partnership approach to working with local authority partners, NHS services, multidisciplinary teams, therapists and other community bodies involved in a person’s care.

At Prestige Nursing & Care in Derby we have formed a strong relationship with the teams at Derby City, Erewash Borough, Derbyshire County, Nottinghamshire Country and Nottingham City Social Services, and work closely with Derby, Derbyshire, Nottingham and Nottinghamshire Clinical Commissioning Groups (CCGs).  These working partnerships mean we are well-informed so we can provide continuity of care, ensuring the right care plan is in place at the right time, whilst responding efficiently to changing needs and strengthening care provision when required.

We work closely with hospital discharge teams at Queen’s Medical Centre, Derby Royal Hospital, Derby London Road and Nottingham City Hospital enabling efficient and effective discharge for people with the right care in place.

I joined Prestige Nursing & Care in 2010 as a part time worker in the Leicester Branch. In 2018 I moved to the Derby Branch as the Registered Care and Compliance Manager. Over the years I have carried out numerous roles including Ad Hoc and On Call, Care Planning, Field Care Supervising and Recruiting. I thoroughly enjoy my job and couldn’t imagine doing anything else.

I have worked in care since I finished my A-Levels and have previously worked for the NHS for 5 years in different wards. I have a NVQ level 2, 3 and 5 in Health and Social Care and am close to completing my Level 5 Apprenticeship in Operational and Departmental Management.

I first joined Prestige in 2006 at the age of 55 when I retired from full time work. I left some time after and then joined Prestige again in 2016. 

In 2018 I took on the Role of Branch Nurse, which then became Regional Nurse. I love my Role.  I mostly support My Care Colleagues, predominantly the ones who provide Complex Care.  I have the pleasure of meeting our Clients and their Families in their own homes.  My role is very varied, and no two days are the same.  I have become Teacher, Assessor, Manager, Mentor and Guide.  It is like all of my skills have been drawn together in this one ‘Meant to Be’ Role.  After 0ver 30 years as a Registered Nurse I have the opportunity to pass on some of my knowledge and Skills before age finally catches up with me.  It is a Privilege to do what I do, and I consider myself to be very lucky.

Going back to the start of my care career I commenced my Training at the age of 37 and in 1990 became a Registered General Nurse.  In 1998 I gained my  ‘City and Guilds Further and Adult Education Certificate,’ and followed that a year later with ‘The Counselling and Psychology Central Awarding Body, Certificate in Counselling Skills’. I have undertaken many courses since then, and have recently completed the Parkinson’s UK ‘Train the Trainer Course’.

My first post after qualification was General Surgery, then in 1992, I moved to a Hospice as I wanted to provide Quality Person Centred Care for people at at the end of their life.

In 2006 at the age of 55 I retired from Full Time work and it was then that I first joined Prestige. Due to other employment opportunities with Derby and Derbyshire Health Trusts I ended up leaving a while after.

In 2009 I  was approached by a Company called ‘Handle With Care’ who provided Training for several Domiciliary Care Companies and Care Homes, and was asked to join their Team of Trainers and for a while I worked for them.

In 2016, I decided to go back to direct patient care before I got too old to manage the physical requirements, and came back to Prestige. The Derby Branch were concentrating predominantly on Homecare with some Healthcare shifts in Nursing homes, so I worked mostly as a Health Care Assistant.  I did some work with Whizz Kids, both as a Nurse and a Support worker, and loved every minute.

I have been a paediatric nurse for over 16 years, initially working within a children’s teaching hospital rotating within the areas of burns and plastic surgery, orthopaedics and general medical/ neurology, nursing children from infancy up to 17 years old.

In 2005 I joined a team on a Lead Level 3 Neonatal Intensive Care Unit. Over the next 14 years, I worked as part of specialist team of health professionals, caring for premature infants from 23 weeks gestation and sick infants requiring many forms of intensive care and specialist treatments, surgical intervention or palliative care. I assisted with clinical research programmes, supported and delivered teaching to parents/guardians to provide complex care for their child such as tracheostomy care, enteral feeding, oxygen therapy and suction training in preparation for their discharge home. I became a student nurse mentor supervising, teaching and assessing clinical skills and competencies at the end of the training programme.

I joined Prestige in March 2020 as a Regional Nurse in the East Region leading complex care packages for children working in partnership with multi-disciplinary teams to meet the needs and expectations of children and their families, all whilst maintaining the high standard of care required and ensuring best practice.

I joined Prestige Nursing & Care In January 2022. I qualified as a Registered Nurse with a BA in Professional Studies in Nursing from Queen Margaret University in Edinburgh in 1988, and attained an MSc in Nursing from Manchester University in 2004. I spent 33 years working in the NHS, 31 years of which was as a District Nurse where I was also a qualified Community Practice Teacher/Assessor and nurse prescriber.

In 2017, after a short stint working with NHS 111, I started work in the social care sector, firstly as a Regional Nurse, before moving into an Operational Manager position.

I have a passion for learning and development and am keen to working with my colleagues across Prestige to further develop this function and support our teams with reaching their potential.

I qualified as a Registered Nurse in 1994 at East Surrey Hospital and as a Paediatric Nurse in 2000 at Kingston and St George’s University Hospitals. I have gained a wealth of experience in a variety of settings firstly in adult surgical nursing, then working in the outback of Australia providing paediatric nursing in the community, paediatric care in a hospice and on general paediatric wards. My areas of interest include oncology and high dependency. I have always enjoyed teaching and have been a mentor to many colleagues.

I joined Prestige in April 2018 where I have been given the opportunity to develop in my role. As the Paediatric Clinical Lead and Paediatric Safeguarding Lead for the company, I am particularly proud of the work we have done to further develop our paediatric training.

I qualified as a Registered Adult Nurse in 2011 at Royal Preston Hospital through the University of Central Lancashire. I have gained a wealth of experience in a variety of different settings, firstly in Intensive Care at Preston then through multiple different hospitals across the North West. I then moved into Renal care before training as a Disability Analyst assessing for Personal Independence Payments. Following this was the start of my community based nursing journey. I worked in a complex care nursing unit for 12 months then moved into Complex home care.

I began working for Prestige Nursing & Care in January 2022 and love what I do. I provided clinical expertise, training and professional leadership to the Blackpool team, to ensure the safe planning and delivery of high quality care to adults with complex needs within their homes and community settings. I take great pride in my work and my aim is to always ensure person centred, holistic care is delivered to our clients and families.

As of February 2023 I am now the Mobilisation Complex Nurse Specialist covering England and Scotland.

My areas of interest include Complex Care, in particular Tracheostomy management and care. I have always enjoyed teaching and mentoring new staff and students within my career. My aim at prestige is to ensure support and training is always available to our carers and team members.

COST OF OUR CARE IN DERBY

The cost of our care is dependent on the level of care and support you require.  If you are receiving our hourly care service, the overall cost will be determined by the number of visits and frequency of visits to you at home. Our experts will conduct a comprehensive assessment of your needs at your home, or in hospital if you are awaiting discharge to truly understand what you need and therefore how much care at home will cost.

We know that the cost of our care in Derby is very competitive when compared with other home care providers in the area.  It is worth noting that when comparing the cost of home care with a care package in a care home, that the fee you pay for home care is actually for the care and support you receive.  In a care home your weekly fee mainly covers your accommodation, food and social activities with the care package being only a small part of what you pay.

You may be entitled to either social care funding, healthcare funding or Government funding that could contribute to the cost of care at home.  If you are faced with having to finance your own home care (known as self-funding care), there are initiatives available to you, including care fees annuities or equity release schemes that could provide the funds you need for your care and support.
