WHAT IS CASE MANAGEMENT FOR BRAIN INJURIES?
Case management refers to a comprehensive approach to planning, coordinating and managing the care and support required by people with complex clinical needs, like those with acquired brain injuries. Case management focuses on improving the person’s overall well-being and achieving the best possible health outcomes whilst optimising the available budget.
Prestige Nursing & Care is a fully managed and CQC-regulated provider of home care services. We have built a strong reputation as a reliable partner for Case Managers throughout the country, collaborating closely to offer their clients bespoke and outcome-driven care plans tailored to the unique needs of each individual.
Our focus is on improving quality of life for your clients. We work with you, your client and their families to:
- Define the individual’s unique health, social and emotional needs
- Create a bespoke plan of support guided by the individual’s goals
- Monitor the care provided to ensure it is always improving health outcomes
- Collaborate with family and support networks, whenever appropriate
We understand the need Case Managers have for regular reporting, and we have developed unique reporting measures to ensure collaborative working with our community partners.
EXPERT NURSE-LED CARE COMPLEX CARE PACKAGES
We understand that commissioners of complex care packages are looking for a professional, safe, reliable and effective clinical approach to providing care and support. Whether we are providing paediatric care to a young child or supporting an adult living with a brain injury, through to older adults living with complex neurological conditions or other physical disabilities, we ensure that the right care is in place, at the right time.
Working closely with our professional partners, our clients and their families, our team can plan and deliver safe and effective complex care to our clients. We are experienced in identifying specific care needs, monitoring changes and planning the most appropriate care provision whilst ensuring all our carers have the skills they need to deliver high standards of nursing care.
We know through our existing partnerships with the Integrated Care Board (ICB), formerly Clinical Commissioning Groups, that this clinical excellence provides reassurance and drives efficiencies in working and delivers best practice clinical care to those we care for. Our nursing teams are experienced in providing high levels of reporting to ICB clinical teams, saving you valuable resources.
We also work collaboratively with other healthcare professionals, multidisciplinary teams, therapy providers and community support services involved in our client’s care to ensure an integrated and holistic approach to our brain injury care. The level of care provided is planned in partnership with everyone involved in a client’s care. We know through our experience this ensures goals are reached and outcomes are improved.

HOW WE PROVIDE COMPLEX CARE FOR BRAIN INJURIES
Our trusted complex care is nurse-led and can be provided in the following ways:
- Hourly care or visiting care is flexible and can be provided for as many hours as required throughout the day or week to ensure our clients are never without the support they need.
- Respite care for those needing a short-term arrangement after hospital discharge to rehabilitate or when a family carer needs a break from their caring role.
- 24-hour care is for people with substantial care needs who need round-the-clock support to live well at home. A team of dedicated carers will take turns being with you throughout the day and night, so you’re never left without the support you need.
OUR APPROACH TO BRAIN INJURY CARE AT HOME
At Prestige Nursing & Care, we provide a care and support package that enables you to live a happy and fulfilled life in the safety and comfort of your own home. No matter your age, the brain injury care and support we provide will be guided by you and tailored to your needs, giving you choice over how you live your life and the support you want.
Our focus is for you to have as much enjoyment in life and our carer will support you with a care plan for brain injury that means you can enjoy activities and as much socialisation as you wish. They will actively encourage you to learn new skills and engage in new hobbies to make life stimulating and meaningful.
REABLEMENT CARE
For clients living with a brain injury who need to relearn skills, our carers provide goal-focused care at home. This involves intensive time-limited assessment and therapy intended to reduce the need for a long-term care arrangement or readmission to hospital.
ENABLEMENT CARE
We provide support to people living with a brain injury who need to learn new skills to improve the overall quality of their life with as much independence as possible. Our carers are focused on enabling a person to do as much for themselves as possible with the right support, as opposed to just doing it for them. We know this means people can live a more fulfilled and purposeful life.
REHABILITATION
We assist outcome-based rehabilitation at home following a life-altering incident like a brain injury that supports an individual with mobility, physical development and mental health well-being. We adopt a multi-disciplinary approach to the service we provide, working in collaboration with other healthcare professionals, including physiotherapists, occupational therapists and speech and language therapists. This ensures a high-quality, holistic and responsive approach to the brain injury care we provide.
PROMOTING POSITIVE MENTAL HEALTH
We know how much a brain injury can affect a person’s mental health, impacting how they function day-to-day, their relationships, physical health and ability to enjoy life. Our expert care team encourages healthy behaviours that promote a positive mood and build self-esteem and can identify and react to symptoms early by effectively monitoring a person’s well-being.
We work collaboratively with our clients and their families to develop a thorough care and support plan that accurately reflects a person’s holistic needs. We also adopt a partnership approach to working with local authority partners, NHS services, multidisciplinary teams, therapists and other community bodies involved in a person’s care. This means we are well-informed to provide continuity of care, ensuring the right care is in place at the right time, responding efficiently to changing needs and strengthening care provision when required.
