WHAT IS PAEDIATRIC CARE?

Paediatrics refers to the area of medicine that manages medical conditions that affect infants, children and young people up to the age of 18. Paediatric care is holistic and considers not just the symptoms of the condition or disability a child is living with, but covers other factors including mental well-being, socialisation and educational development. The goal of paediatric care is to allow a child to live as normal a life as possible.

Paediatric care can be provided in several different settings, including general hospitals, community settings and in a child’s own home with care provided by a specialist care provider, like Prestige.

Nurse-led paediatric care at home is led and managed by a registered nurse who is highly experienced. The registered nurse guides and oversees the clinical care provided by a trained carer.

Nurse-led paediatric care requires specialist expertise and knowledge to ensure a high-quality service is delivered with clinical intervention that is both safe and effective, whilst positively enhancing the experience for the person receiving care. The nursing care service provided must be able to identify changes in health and evolve the care provided as needs change.

HOW WE PROVIDE PAEDIATRIC CARE AT HOME

Our trusted and established paediatric care service can be provided in the following ways for those needing paediatric care:

  • Hourly care or visiting care for those who need support with tasks of daily living or clinical care at certain times during the day. Paediatric care at home can be provided from a 60 minute visit up to as many hours/visits throughout the day as you require, one day or several days of the week. For those who have complex needs, hourly care can be provided over 24 hours.
  • Respite care is a short-term arrangement after hospital discharge to rehabilitate or when a family carer needs a break from their caring role. It is also an ideal service for families who wish to assess whether care at home is the right care choice for them.

Jake is a baby with complex medical needs. He was born prematurely and had been in hospital since birth. He requires ventilation with oxygen via tracheostomy 24-hours a day.

Whilst Jake’s parents had received training to meet his needs, they could not provide the 24-hour care that Jake required. His dad had to return to work and his mum was expecting their second child.  Both parents were extremely keen to get Jake home and into a routine before the arrival of the new baby.

Jake required a team of fully trained carers to meet his complex needs at home. Training for the carers needed to be carefully coordinated in a timely and efficient manner, to ensure that all carers involved in providing care to Jake were fully competent and able to meet his needs.

Prestige Nursing & Care identified carers who completed the required training.  They then worked shadow shifts at the referring hospital alongside our experienced registered nurse prior to Jake’s discharge. This ensured both Jake and his parents became familiar with the care team, enabling them to be reassured about their level of competence.  Training completed by the team included tracheostomy care and management, invasive ventilation, emergency procedures, nebuliser and oxygen therapy, enteral feeding and administration of medication.

The Prestige team worked closely with the healthcare professionals involved in Jake’s discharge from hospital.  They also worked with the multi-disciplinary team, which included the community children’s nursing team representative, a physiotherapist, occupational therapist, speech and language therapist and a dietitian.  This enabled a holistic approach to the development of his paediatric care package.  A comprehensive care plan was created with realistic goals and his discharge date was set.

Prestige worked in collaboration with local services to ensure that all essential equipment and supplies would be available following discharge.  A plan for follow up and escalation of treatment was written and shared with all relevant healthcare professionals and the Prestige care team.

Jake was discharged within six weeks of the first multi-disciplinary team meeting and is receiving high levels of support from the Prestige team in his own home, with his family around him.  On discharge our registered nurse attended his home to ensure all the carers had the support they needed, and his parents were reassured about the service.    Jake is now living a healthy and happy life at home and has welcomed the arrival of his baby sister.

COMPETENT AND COMPASSIONATE PAEDIATRIC CARERS

Jessica  has highly complex health needs which require constant intervention from highly skilled carers.

Jessica’s nursing care needs increased after her tracheostomy and she was prone to seizures, so her parents were exhausted. They needed trusted and reliable nursing care to brighten up her life. Jessica is very sociable with a good sense of humour. As her condition is life-limiting, it is vital to her parents that Jessica’s life is full of good memories and experiences.

After being repeatedly let down by other care providers Jessica’s case manager recommended Prestige Nursing & Care to her parents, Richard and Jane. The carers chosen for Jessica were trained by our paediatric nurse to ensure they had the level of skills and aptitude to meet Jessica’s needs.  Our nurse approved the care training and once competent, the carers worked closely with Richard and Jane to deliver Jessica's care.

Jessica has care from Prestige in the mornings and evenings, with double-up care twice a week so Jessica’s parents can go out.  Paediatric care is also provided during the day at weekends, and during the night when needed.  Specific clinical interventions provided by the Prestige carers include tracheostomy care and management, button feed PEG, oxygen monitoring and administration, administering and managing epilepsy and opiate medication, hoisting and personal care.

Richard and Jane could not be happier with Jessica’s care, and value the support provided to all the family.  Richard comments: “We’re all part of the team. The carers are very competent and well trained.”

Jane says: “Prestige has taken the pressure off us. They are absolutely brilliant.  They are always reliable and I cannot thank them enough for their support.”

Jessica enjoys fantastic days out like visits to the zoo and sea life centre where she loves the animals and sounds.  She has even been on a big wheel at a fairground! When she was recovering from a broken leg, the carers enabled her to maintain her independence and she continued to go out every day.

Richard and Jane are completely relaxed to leave Jessica with her care team, and can now enjoy occasional weekends away as well as regular nights out. Jessica, her parents and care team are all looking forward to a summer holiday together.

We work collaboratively with our clients and their families to develop a thorough care and support plan that accurately reflects a person’s holistic needs. We also adopt a partnership approach to working with local authority partners, NHS services, multidisciplinary teams, therapists and other community bodies involved in a person’s care.

This means we can provide continuity of care ensuring the right care is in place at the right time, whilst responding efficiently to changing needs and strengthening care provision when required.

We work in partnership with other healthcare professionals, multidisciplinary teams, commissioning groups and other therapy providers involved in our client’s care to ensure an integrated and holistic approach to the paediatric care we provide.

Once a child reaches adulthood at 18 years old, we work closely with all professionals involved in their care to ensure an effective transition into adult care services that offers continuity and consistency of care.

COST OF PAEDIATRIC CARE

The cost of complex care is dependent on the level of care and support you require.  If you are receiving our hourly nurse-led care the overall cost will be determined by the number of visits and frequency of visits to you at home. Our experts will conduct a comprehensive assessment of your needs at your home, or in hospital if you are awaiting discharge to truly understand what you need and therefore how much complex care at home will cost.

We know that our cost of professional nurse-led care is very competitive when compared with other home care providers.  It is worth noting that when comparing the cost of paediatric care with a nursing care package in a nursing home, that the fee you pay for paediatric home care is actually for the care and support you receive.  In a nursing home your weekly fee mainly covers your accommodation, food and social activities with the care package being only a small part of what you pay.You may be entitled to either healthcare funding, social care funding or Government funding to contribute to the cost of complex care at home.  If you are faced with having to finance your own care at home (known as self-funding care), there are initiatives available to you, including care fees annuities or equity release schemes that could provide the funds you need for your care and support.
