Post hospital care

Navigating the transition from hospital to home can be a challenging and overwhelming experience for many individuals. The need for ongoing support with medication management, wound care, and rehabilitation, often necessitates intensive support, especially in the initial stages of returning home.

At Prestige Nursing & Care, we understand the importance of providing personalised care services to facilitate a smooth and comfortable transition from hospital to home. Our dedicated team tailors our home from hospital services to meet your individual needs, ensuring that you receive the right level of support precisely when you need it.

With our compassionate and experienced carers by your side, you can rest assured that you’ll receive the highest quality care to promote your post hospital discharge recovery and well-being, all in the comfort of your own home.

What is post hospital care?

Post-hospital care is all about bringing comfort, support, and peace of mind to individuals as they transition from hospital to the familiar surroundings of their own home.

Our post-hospital care services are designed with your well-being in mind. We’ll work closely with you and your loved ones to create a tailored support plan that meets your unique requirements and help you feel at ease throughout your recovery journey at home.

Some of the duties and responsibilities our carers can help with include:

  • Meeting the client at the hospital and/or making arrangements to ensure they get home from the hospital in a safe and timely manner.
  • Developing a care plan that will incorporate instructions from the medical staff.
  • Ensuring that the client has all their medication, patient notes, instructions etc.
  • Preparing the home for the client’s arrival and settling the client in.
  • Setting up and coordinating any required exercise programme, recovery programme or visits with other healthcare professionals.

With our compassionate carers by your side, you can rest assured that you’ll receive the care and attention you deserve. Whether you’re recovering from an illness, surgery, frailty, or infection, our dedicated team is here to support you every step of the way, ensuring your safety, comfort, and overall well-being.

Planning ahead before hospital discharge

When someone is getting hospital care that’s been planned in advance, planning for their discharge should start before they go into hospital. If the visit is unplanned, then within 24 hours of a person’s arrival in hospital a treatment plan will be developed, which will include proposals for the discharge process.

Each hospital will have its own policy for discharging patients and you can speak to the ward manager or professionals in charge of you or a loved one’s care in order to obtain a copy. This document will help you to start thinking about what will happen following a hospital stay.

Will I be assessed for help at home?

When it comes to receiving help at home after a hospital stay, hospitals often utilise a “discharge to assess” or “home first” approach to discharge planning. This means that instead of conducting a care needs assessment before you leave the hospital, an initial assessment is carried out to ensure your safe discharge. Your long-term needs will then be assessed after a period of recovery at home.

During your hospital stay, staff will work to ensure that support services and any necessary home adaptations are arranged before your discharge. However, if home adaptations are needed, interim care options may need to be considered while waiting for these adaptations to be completed.

It’s essential to clarify any costs associated with the services being offered upon discharge, as different areas may have varying rules and regulations regarding funding for home care services.

Once you’ve had time to recover at home, a comprehensive assessment of your long-term needs, often referred to as a care needs assessment, will be conducted. Following this assessment, you’ll have the opportunity to actively participate in the creation of your care plan. Additionally, you should be assigned a named person whom you can contact if you have any questions or concerns about your care.

What types of post hospital care are available?

After a hospital stay, various types of post-hospital care are available to support individuals during their recovery process. Here are some common options:

Intermediate care

Intermediate care provides short-term support and rehabilitation services to people who have been discharged from the hospital but are not yet ready to return to their usual living arrangements. This type of care aims to promote independence and prevent unnecessary hospital readmissions by providing intensive support and therapy.

Convalescent care

Convalescent care, also called reablement care, focuses on helping individuals regain their independence and functional abilities following a hospital stay or illness. It involves personalised support and rehabilitation services tailored to the individual’s specific needs and goals. Reablement care aims to improve confidence, mobility, and overall well-being to facilitate a successful return to independent living

Nurse-led care

Nurse-led care – sometimes called complex care – is nursing care which is led and managed by a highly experienced registered nurse. This complex care nurse will guide and oversee the clinical care provided by a trained carer. As it may include complex interventions and related care, home nursing demands specialist expertise and knowledge to ensure a high-quality, safe, and effective service.

Why choose Prestige Nursing & Care?

See for yourself why our clients choose Prestige Nursing & Care to provide the dedicated post hospital care they need:

A fully managed service

Prestige Nursing & Care is not an agency. Unlike a home care agency, we provide a fully managed and regulated care at home service. This means our clients, their families and our professional partners can be reassured that the care and support we provide is of the very highest quality. Families can focus on spending quality time together rather than managing the care arrangement.

CQC regulated care

Our service is inspected by the Care Quality Commission (CQC) in England and the Scottish Care Inspectorate (SCI) in Scotland. We are proud that 96% of our inspected services have been rated as providing a ‘Good’ service, with 4% rated ‘Outstanding’ in England.

Our accredited service for complex care, which meets local and national quality standards and ensures a consistent approach to the care we provide, provides peace of mind and reassurance that our service is of the highest quality.

Rigorous recruitment & training process

Our commitment to excellence starts with our care team. Our rigorous recruitment process ensures only the most qualified and dedicated individuals join our team. Our carers undergo comprehensive training to ensure they possess the skills, knowledge, and compassion required to provide exceptional care. All of our carers have at least six months of care or health sector experience before being placed with a client or community partner.

Complex & specialist care

Our service is nurse-led and our carers are expertly trained to provide care for complex, terminal and other medical conditions. They are supported by a registered home nurse who provides unrivalled monitoring and supervision of our clinical care. With this leadership and oversight, carers can perform many delegated healthcare tasks and interventions at home.

Care management local to you

Our responsive and reliable local nurse-led service allows us to put a care pathway and plan in place quickly and, in many cases, within 24 hours. We work in partnership with healthcare professionals, Case Managers, families and personal injury lawyers to ensure we have an in-depth understanding of our client’s needs. Our local registered nurse will conduct a comprehensive assessment of your needs at home or in any specialist care setting, enabling a plan of care and support to be developed that meets all your needs.

Our approach to care planning, whether it is for an hourly, respite or live-in care service, ensures we capture all your preferences and wishes so you can live a contented life in the comfort of your own home. We take time to find the nursing carer that would best meet your nursing needs, making sure they are closely aligned with how you wish to live your life.

Once care is in place, our local teams made up of care and clinical experts, provide you, your family and our carer with an abundance of ongoing support. This gives you reassurance that you or your loved one is receiving the very best complex care at home.

Cost of post hospital care

The cost of post hospital home care depends on the level of care and support you require. If you are receiving our hourly care, the overall cost will be determined by the number of visits and frequency of visits to you at home. Our experts will conduct a comprehensive assessment of your needs at your home or in hospital if you are awaiting discharge, to truly understand what you need and how much care at home will cost.
