- Meeting the client at the hospital and/or making arrangements to ensure they get home from the hospital in a safe and timely manner.
- Developing a care plan that will incorporate instructions from the medical staff.
- Ensuring that the client has all their medication, patient notes, instructions etc.
- Preparing the home for the client’s arrival and settling the client in.
- Setting up and coordinating any required exercise programme, recovery programme or visits with other healthcare professionals.

Planning ahead before hospital discharge
When someone is getting hospital care that’s been planned in advance, planning for their discharge should start before they go into hospital. If the visit is unplanned, then within 24 hours of a person’s arrival in hospital a treatment plan will be developed, which will include proposals for the discharge process.
Each hospital will have its own policy for discharging patients and you can speak to the ward manager or professionals in charge of you or a loved one’s care in order to obtain a copy. This document will help you to start thinking about what will happen following a hospital stay.
Will I be assessed for help at home?
When it comes to receiving help at home after a hospital stay, hospitals often utilise a “discharge to assess” or “home first” approach to discharge planning. This means that instead of conducting a care needs assessment before you leave the hospital, an initial assessment is carried out to ensure your safe discharge. Your long-term needs will then be assessed after a period of recovery at home.
During your hospital stay, staff will work to ensure that support services and any necessary home adaptations are arranged before your discharge. However, if home adaptations are needed, interim care options may need to be considered while waiting for these adaptations to be completed.
It’s essential to clarify any costs associated with the services being offered upon discharge, as different areas may have varying rules and regulations regarding funding for home care services.
Once you’ve had time to recover at home, a comprehensive assessment of your long-term needs, often referred to as a care needs assessment, will be conducted. Following this assessment, you’ll have the opportunity to actively participate in the creation of your care plan. Additionally, you should be assigned a named person whom you can contact if you have any questions or concerns about your care.
