Your Role
You will play a key part in the success of our Chichester branch by coordinating care delivery, supporting frontline teams and maintaining high standards of quality and compliance.
This role is ideal for an experienced care coordinator or planner who is proactive, solution-focused and confident working in a busy, people-centred environment.

Main Responsibilities
- Ensure all scheduled client visits are safely covered each day.
- Prepare and finalise rotas at least four weeks in advance.
- Allocate care staff according to client needs, preferences, skills and continuity requirements.
- Organise and coordinate quality visits for Field Care Supervisors and Field Care Managers.
- Liaise with Branch Nurses regarding complex care assessments, training and competency checks.
- Keep care teams informed of changes to client needs, risks and care plans.
- Maintain strong and professional relationships with clients, families and frontline colleagues.
- Respond promptly and professionally to queries, concerns and changes in care requirements.
- Work alongside care planners to coordinate services within an allocated geographical area.
- Collaborate with healthcare professionals, social workers, case managers and multidisciplinary teams.
- Ensure records, office systems and operational procedures are accurate and compliant.
- Prepare operational and management reports as required.
- Identify local business development opportunities alongside the Branch Manager.
- Promote excellent internal customer service and effective teamwork.
- Participate in the on-call rota, including approximately one weekend in four.
- Support neighbouring branches when required.
- Contribute flexibly to the day-to-day running of the branch.
- Carry out any other reasonable duties appropriate to the role.
Key Accountabilities
- All client visits are covered safely and effectively.
- Rotas are completed and communicated at least four weeks in advance.
- Care staff are matched appropriately to client needs and preferences.
- Changes to rotas and care plans are communicated promptly to all relevant parties.
- Positive relationships are maintained with clients, families, colleagues and external professionals.
- Records and systems remain accurate, complete and compliant.
- Service quality, continuity and client satisfaction are consistently supported.
Essential Requirements
- Previous experience in a care coordination, scheduling or care planning role.
- A proactive and solution-focused approach.
- Strong organisational and time-management skills.
- Excellent communication and interpersonal skills.
- Ability to work calmly and effectively in a fast-paced environment.
- Confidence using rota, scheduling, CRM or care management systems.
- Ability to manage competing priorities and respond to urgent staffing issues.

- Willingness to participate in the on-call rota.
Desirable Requirements
- Full UK driving licence and access to a vehicle.
- Experience coordinating complex or nurse-led care packages.
- Confidence promoting branch services to potential clients and professional stakeholders.
- Awareness of regulatory requirements and quality standards within home care.
- Passion for continuous improvement and service excellence.
