We offer complex nursing care, led by an experienced registered nurse, provided in the client’s own home/ community setting and delivered by a nurse, heath care assistant or support worker.

Our client needs, which vary, mainly arise from physical disability, long term conditions, neurological impairment, sensory impairment, learning disability and autism and mental health issues.

We work in partnership and collaborate with those health-care professionals already involved; we value the importance of integrated and joined-up care provision.

We are looking for a Registered Nurse, qualified either as a RMN or RNLD, who is a dynamic and independent Nursing professional with solid challenging behaviour experience and a flexible, enthusiastic, and can-do attitude to join our Bristol team supporting a gentleman with complex behaviour and social needs clients based in the Kingswood area.

This role requires an experienced registered nurse/ behaviour support practitioner or someone with a demonstrated background in challenging behaviour to provide clinical guidance and leadership to an established team of Support Workers to improve the quality of life of our client by facilitating community access, reviewing PBS and Care plan, attending MDT meetings as required and supporting with medication management.

ROLE PURPOSE

The purpose of this role is to provide clinical expertise and professional leadership to a specific client in his own home, to enable the planning and delivery of safe and high-quality complex care provision.

  • To be accountable for and lead on the clinical components of care delivery
  • To risk assess individual client activities of daily living and to continually provide clinical input, expertise and advice relating to care-planning and delivery across the pathway and evaluate outcomes
  • Attend meetings and/or liaise with MDT, commissioners, case managers and family etc
  • To act as a clinical role model and leader for all members of staff within the package
  • To facilitate and deliver an ongoing programme of clinical knowledge/skills-training and competency assessments to support workers to enable them to competently deliver care with minimal supervision
  • Maintain standards, proactively manage clinical risk, complaints and play a pro-active role in quality improvement
  • Promote compliance and adherence to internal policies and standards
  • Keep abreast of national standards and requirements surrounding adult services, and relevant specialism
  • Commitment to improving client/family experience at every step of the care pathway.

KEY ATTRIBUTES/KNOWLEDGE/SKILLS/EXPERIENCE

QUALIFICATIONS AND EXPERIENCE AND SKILLS (D-Desirable, E-Essential)

  • RNLD/RMN/RN with current NMC registration E or a behaviour support practitioner D
  • Demonstrated experience of working with patients/clients with challenging behaviour E
  • Experience of working with individuals with sensory impairment E
  • Significant experience of care management, including relevant clinical experience, in the community or hospital setting with evidence of professional development E
  • Management of risk and health and safety E
  • Experience of safeguarding adults and children procedures and ability to implement E
  • Must have full UK driver’s license and be confident to drive a van E
  • Effective communication skills – verbal and written E
  • Ability to work on own initiative as well a part of a team – self-motivated E
  • Coaching and mentoring skills, teaching qualification D
  • Evidence of success in leading/managing significant and sustained change D
  • Effective people management skills E
  • Ability to negotiate, influence, escalate and seek advice E
  • Experience in interpreting and analysing data and meet deadlines D
  • Excellent IT and presentation skills E
  • Demonstrate knowledge of the theory and application of clinical governance E
