WHAT IS NURSE-LED CARE?

Nurse-led care – sometimes called complex care – is nursing care which is led and managed by a highly experienced registered nurse This complex care nurse will guide and oversee the clinical care provided by a trained carer.

The goal of complex care is to enhance the life of the person receiving care. Because it may include complex interventions and related care, home nursing demands specialist expertise and knowledge to ensure a high-quality, safe, and effective service.

As part of our nursing care service, we assess care needs and plan the care and support required. We continually monitor and evaluate outcomes, so the care provided meets changing needs.

Nurse-led care includes:

  • Establishing and agreeing on clinical care needs
  • Undertaking risk assessments
  • Developing a plan of care working with the client and family to set expectations
  • Providing hands-on care at home
  • Evaluating the care provided, ensuring the right care is delivered at the right time so a person’s needs are met – every time
  • Providing advice and support to the client and their family
  • Working in collaboration with other healthcare professionals involved in a person’s care

Our service is nurse-led and carers are trained to provide care and support for complex, terminal and other medical conditions that impact how you live your life. We work collaboratively with other healthcare professionals, multidisciplinary teams, commissioning groups and other therapy providers involved in our client’s care to ensure an integrated and holistic approach to nursing care we provide. Family involvement is central to our approach to providing nurse-led care at home and our care teams will ensure they receive high levels of support, as and when they need it. The level of care provided is planned in partnership with everyone involved in a client’s care. COMPETENT AND COMPASSIONATE NURSE-LED CARERS

When you choose Prestige Nursing & Care you can be reassured that the carer who cares for you or your loved one is experienced and expertly trained to provide the support you need, to the high standards you should expect.

If you are receiving complex care at home, the carers we choose to support your needs will have at least six months care or health sector experience before they come to care for you. In addition to completing our induction training programme, which goes above mandatory requirements they are trained by our registered nurses, clinical experts and other healthcare professionals involved in providing care to a specific client.  This hands-on nurse training means our carers can competently undertake a range of clinical interventions.

We adopt a bespoke approach to training that ensures our carers have the knowledge, skills and aptitude to deliver high-quality complex care that meets the very specific needs of our clients.  Through our unrivalled supervision and clinical oversight carers can adapt their approach and evolve the care they provide to meet changing needs.

CLINICAL EXCELLENCE AT PRESTIGE NURSING & CARE

Clinical excellence is at the heart of our culture and approach to service delivery at Prestige Nursing & Care. Our nurse-led home care service is monitored and supported by our experienced regional nursing teams.  Our team of qualified regional nurses are registered with the Nursing and Midwifery Council (NMC). Each holds a registered nursing qualification in at least one nursing specialism, including general adult nursing, learning and disability nursing, mental health nursing and children’s nursing. They enable the planning and delivery of safe and effective complex care to our clients, that is focused on ensuring the best possible care experience for our clients and their families.

Our nurses work in partnership with our care co-ordinators, carers, clients and families to identify specific care needs and plan the most appropriate care provision, whilst ensuring all our carers have the skills they need to deliver high standards of nurse-led care. They also provide families with on-going advice and continual support to ensure they feel reassured about the service we provide.

SUPPORTING JESSICA WITH PAEDIATRIC CARE AT HOME

Jessica  has highly complex health needs which require constant intervention from highly skilled carers.

Jessica’s nursing care needs increased after her tracheostomy and she was prone to seizures, so her parents were exhausted. They needed trusted and reliable nursing care to brighten up her life. Jessica is very sociable with a good sense of humour. As her condition is life-limiting, it is vital to her parents that Jessica’s life is full of good memories and experiences.

After being repeatedly let down by other care providers Jessica’s case manager recommended Prestige Nursing & Care to her parents, Richard and Jane. The carers chosen for Jessica were trained by our paediatric nurse to ensure they had the level of skills and aptitude to meet Jessica’s needs.  Our nurse approved the care training and once competent, the carers worked closely with Richard and Jane to deliver Jessica's care.

Jessica has care from Prestige in the mornings and evenings, with double-up care twice a week so Jessica’s parents can go out.  Paediatric care is also provided during the day at weekends, and during the night when needed.  Specific clinical interventions provided by the Prestige carers include tracheostomy care and management, button feed PEG, oxygen monitoring and administration, administering and managing epilepsy and opiate medication, hoisting and personal care.

Richard and Jane could not be happier with Jessica’s care, and value the support provided to all the family.  Richard comments: “We’re all part of the team. The carers are very competent and well trained.”

Jane says: “Prestige has taken the pressure off us. They are absolutely brilliant.  They are always reliable and I cannot thank them enough for their support.”

Jessica enjoys fantastic days out like visits to the zoo and sea life centre where she loves the animals and sounds.  She has even been on a big wheel at a fairground! When she was recovering from a broken leg, the carers enabled her to maintain her independence and she continued to go out every day.

Richard and Jane are completely relaxed to leave Jessica with her care team, and can now enjoy occasional weekends away as well as regular nights out. Jessica, her parents and care team are all looking forward to a summer holiday together.

HOW WE PROVIDE NURSING CARE AT HOME

Our trusted and established care complex care service can be provided in the following ways for someone needing nurse-led care:

  • Hourly care – sometimes called visiting care -is for those who need care and support with daily living or clinical care at certain times during the day. Nurse-led care at home can be provided from  a 30 minute visit, to as many hours/visits throughout the day you require, one day or several days of the week depending on your care and support needs. For those who have complex needs, hourly care can be provided over a 24-hour period.
  • Live-in care for when a permanent care arrangement is needed with greater levels of support around the clock to ensure a person can live well at home. Through our family of leading home care brands in the UK, we are able to offer award-winning live-in care rated ‘Outstanding’ by The Care Quality Commission (CQC) from The Good Care Group.
  • Respite care for those needing a short-term arrangement after hospital discharge to rehabilitate or when a family carer needs a break from their caring role. It is also an ideal service for families who wish to assess whether care at home is the right care choice for them.

Our responsive and reliable local nurse-led service for those with complex care needs means we can put a care pathway and plan in place quickly and in many cases within 24-hours.  We work in partnership with healthcare professionals, case managers, families and personal injury lawyers to ensure we have an in-depth understanding of a clients’ needs.  Our local registered nurse will conduct a comprehensive assessment of your needs at home, in hospital or in any specialist care setting if you are awaiting discharge back to your own home enabling a plan of care and support to be developed that meets all your needs.

COST OF COMPLEX CARE

The cost of complex care is dependent on the level of care and support you require.  If you are receiving our hourly nurse-led care the overall cost will be determined by the number of visits and frequency of visits to you at home. Our experts will conduct a comprehensive assessment of your needs at your home, or in hospital if you are awaiting discharge to truly understand what you need and therefore how much complex care at home will cost.

We know that our cost of professional nurse-led care is very competitive when compared with other home care providers.  It is worth noting that when comparing the cost of nursing care with a nursing care package in a nursing home, that the fee you pay for nursing care at home is actually for the care and support you receive.  In a nursing home your weekly fee mainly covers your accommodation, food and social activities with the care package being only a small part of what you pay.You may be entitled to either healthcare funding, social care funding or Government funding to contribute to the cost of complex care at home.  If you are faced with having to finance your own home care (known as self-funding care), there are initiatives available to you, including care fees annuities or equity release schemes that could provide the funds you need for your care and support.
