When you're having a stay in hospital, some extra support ready for you at home can be just what you need to leave hospital sooner and ease the transition to being back at home. Our caring and professional teams of nurses, care and support workers are on hand to give you just the support you need, from a short daily visit right up to 24-hour specialist nurse cover for more complex needs. And it can be a lot more affordable than you might think.
Everybody's needs are very different, so the service we provide will be tailored to your specific requirements. We can get involved in the care planning process before your admission to hospital, or be on hand when you are ready to leave. We can also arrange care at short notice if necessary.
We'll plan with you and your family the support and care you require from our 4 different levels of service. We will also work closely with your GP, hospital consultant and community health team to ensure you receive the right care at the right time.
The service we offer is very flexible, so it can fit precisely with your needs. We have 4 service levels:
Low Dependency, where clients are largely self-caring, but need companionship and support with daily activities such
as housework, preparing meals, shopping or help with younger members of the family.
Medium Dependency will generally include help with personal care such as bathing, dressing and mobility and supporting the client with their daily living activities.
High Dependency offers assistance with all the above but in addition clients may need help with further elements of
their daily routine such as catheters/stoma care, simple dressings and mouthcare. Health conditions such as dementia,
requiring cognitive care would also be included. Our care staff are specifically trained to carry out care at these differing levels, in conjunction with other health professionals, such as the discharge team, GP and District Nurse.
Nursing Care will be required when the clients health needs are more complex and when administration of medicines is part of the care package. Our Qualified Nurses will be able monitor health and manage healthcare needs such as ventilation, tracheostomy, PEG feeds and other invasive nursing tasks.
Home from Hospital is a valuable service for anyone in need of some extra support after a hospital stay - not just for older people.
Take, for example, the case of a relatively young person whose partner works during the day. Some extra support could
avoid the need for the partner to take time off work. Another typical use would be getting some help in the first few weeks after having a baby. Or help looking after a child while the family are juggling all their other commitments.
